We would like to welcome and introduce you to Optimal Health Medicine. The Center is located at the Arboretum in the Belle Grove Professional Office Park within the “South Charlotte Clinical Associates” building. Please note that our name is not listed on the outside of the building. When you enter the building, take the elevator up to the second floor to Suite G, down the hall on the left. There is a sign that says South Charlotte Clinical Associates and our names are on the list of providers outside the door.

In addition to the completed health history form we also request that you bring any health records and recent labwork which might be helpful during our consultation, as well as, a list of all medications and supplements you are currently taking.

When you arrive at the Center, please take a seat in the waiting area and we will escort you to our office shortly. During your visit, we will review your health history questionnaire; We will listen to your concerns and then take your vital signs and perform a targeted physical exam. We will discuss testing options and initial management plans. At your follow-up appointment, we will review your progress, your test results with an explanation and review a plan consisting of holistic treatment options. This plan will include lifestyle modifications (nutrition, exercise, and stress management) options and suggestions ranging from traditional medicine to complementary treatment therapies, including supplements, if you desire.

We are committed to helping you progress on your journey toward better health and commend you for being proactive in your healthcare!
